I’m Dr. Mahnaz Jafari, and I’ve been practicing dentistry in Sterling, Virginia for almost three decades. One question I hear often enough that I wanted to write about it directly is whether Medicaid will pay for braces if you’re an adult. It usually comes up alongside some confusion about program names — patients ask about “Smiles For Children” and “Cardinal Care Smiles” as if they’re two different things, or wonder if one covers braces and the other doesn’t. They’re actually the same program. Smiles For Children was the original name for Virginia’s Medicaid dental benefit, and it’s now branded Cardinal Care Smiles. Some patients still call it by the old name, and honestly, so do some dental offices — but the coverage rules underneath haven’t changed just because the label did.

This one is focused specifically on orthodontics and braces, because that’s a different question with a different answer, and I think patients deserve a straight answer rather than having to piece it together.
The Direct Answer: No
I’ll give you the answer up front rather than making you read to the end to find it. Orthodontic treatment — and that includes both traditional braces and Invisalign — is not a covered benefit under Cardinal Care Smiles (Smiles For Children) for adults age 21 and older. This isn’t a “sometimes approved, sometimes not” situation, and it isn’t something you can appeal your way into with the right paperwork. It’s a standard, firm exclusion built into the adult dental benefit. I want to be clear about that because I’ve seen patients spend time and energy chasing approvals for something that, under the current rules, simply isn’t available to adults. I’d rather tell you plainly now than have you find out after weeks of phone calls.
Why Braces Aren’t Covered
It helps to understand the logic behind the rule, because it isn’t arbitrary. Virginia’s adult Medicaid dental benefit was built to address medically necessary care — treating pain, infection, and disease. Think exams, cleanings, fillings, extractions, root canals, and dentures. These are treatments that address something actively wrong with your oral health. Straightening teeth with braces or clear aligners, for the vast majority of adults, falls into a different category — it’s considered elective and largely cosmetic, even though I know it doesn’t always feel that way if you’ve lived with crowded or crooked teeth for years. Medicaid programs across the country tend to draw this same line for adults: they’ll cover the dental care that keeps you healthy and out of pain, but not the treatment that primarily changes how your smile looks or feels.
I understand why this can feel frustrating, especially if crowded or overlapping teeth are also making it harder to clean properly, or if you’re dealing with jaw discomfort that seems connected to your bite. But from a program design standpoint, adult dental Medicaid benefits are meant to stretch limited funding across the patients who need urgent, health-related care the most — extractions, infections, and restorative work that can’t wait. Orthodontic treatment, by contrast, is typically a months- or years-long process that’s planned rather than urgent, which is part of why it consistently falls outside the adult benefit in Virginia and in most other states.
The One Narrow Exception
There is technically one exception, and I want to describe it accurately rather than let it sound like a workaround, because it isn’t one. In cases where orthodontic treatment is required as part of orthognathic surgery — that’s jaw surgery to correct a significant skeletal or bite abnormality — braces can sometimes be covered, but only with prior authorization and only when it’s genuinely tied to that surgical treatment plan. This is a rare clinical situation, not a realistic path for someone who simply wants straighter teeth. If you have a jaw condition serious enough to warrant surgical correction, that’s a conversation to have with an oral surgeon and your medical team, not something to pursue as a route to getting braces paid for. For the overwhelming majority of adults asking about braces, this exception simply won’t apply.
What About Kids and Teens?
I’ll mention this briefly since it’s a common follow-up question, even though it’s not really what this post is about. For members under 21, Virginia Medicaid can cover medically necessary orthodontic treatment with prior authorization, when it meets specific clinical criteria — typically significant bite or alignment problems that affect function, not just appearance. That’s a genuinely different benefit structure than what adults have access to, and if you have a child or teenager who might qualify, that’s worth a separate conversation with your dental provider. But for adults 21 and up, that door is closed under the current program rules.
